The Youth Support Program achieves positive change in people’s lives through case management of vulnerable people aged 15+ living or enrolled in school in the Gosford CBD area.
Case managers work together with the young person to develop a case plan to promote an increase in positivity, wellbeing and self-care. This could include ways to increase a connection to community or education, emotional and social development, and mental health support.
Case managers will provide young people with relevant information, advice, referrals, advocacy, guidance, support, and skill development to increase self-esteem, confidence, and experiences.
